Our approach to connected care

Connected Care reaches beyond individual support. It’s about embracing community action and our collective responsibility to uphold each other’s human rights. It’s a framework that recognises the importance of culturally safe services and meaningful participation—because involving people in decisions about their own care is imperative for sustained well-being.

The Connected Care framework is a dynamic support model that pairs each person with a Clinical Therapist and an Allied Health Therapy Assistant. Together, they provide targeted services that align with the individual’s specific needs, goals, and support levels. This approach invites the person and their wider network of supports to participate through shared treatment planning and skill development, ultimately enhancing the person’s therapeutic outcomes.

We understand that mental health, disability, and trauma are deeply interconnected experiences that shouldn’t be addressed in isolation. By bringing together a person’s diverse network of support, we can deliver effective and measurable outcomes, ensuring comprehensive care that respects and responds to the complexity of each person’s experiences.

Connected care in action

When people are supported by a connected care team, meaningful outcomes become possible. Here’s how we do it:

1. Get a referral

2. We’ll reach out

We’ll contact you and collaborate with any other professionals involved in your support network.

3. Make a plan

Together, we’ll create a tailored, flexible support plan designed to meet your needs.

4. Meet your Therapist

You'll meet your Therapist and Allied Health Assistant to explore your goals.

5. Ongoing support

Going forward, you’ll receive coordinated care with our friendly and supportive team.

How to make a payment or claim

We want to make your payments and claims process as easy as possible. We accept Medicare, NDIS, private payments, and private health insurance. For more details on billing and insurance options, please visit our payment information page.

How to make a referral

Complete our online form to make a referral. If you need assistance, feel free to contact our care team. Once we’ve processed the form, we’ll reach out to the person you’ve referred to introduce ourselves and help them get started.
